T1 - Dynamics of the Starr-Edwards ball valve prosthesis

Four cases of acute dysfunction of Starr-Edwards mitral prostheses are recorded. The patients presented with sudden dysponea 4 to 18 months after apparently successful mitral valve replacement. The prosthetic valve dysfunction was caused by thrombus on the bare metal cage of the prosthesis. No warning thromboembolic phenomena had been recorded. Urgent replacement of the valve resulted in the survival of one patient.

The first case of aortic valve replacement with the Starr-Edwards metallic prosthesis was reported in 1965. This prosthesis was also used to replace the mitral valve. However, the mitral replacement with this prosthesis was abandoned because of thrombotic complications. In the aortic position, the Starr-Edwards metallic valve was widely used until the 1970s, when it started to be replaced by the tilting disc metallic valves. This case report demonstrates the impressive durability of the Starr-Edwards prosthesis in the aortic position.

N2 - A 40-year-old woman admitted with heart failure had undergone aortic valve replacement with a Model 2310 Starr-Edwards valve due to aortic regurgitation 33 years previously. She had been followed up for several years, but discontinued follow-up and medication (including Warfarin) for the past 25 years. Echocardiography demonstrated marked dilatation and thickening of the left ventricle, and the peak pressure gradient of the prosthesis was measured as 87.9 mmHg. Under the diagnosis of chronic aortic valve prosthesis-patient mismatch with subsequent severe left ventricular dysfunction, the Starr-Edwards valve was explanted and replaced with a 23 mm St Jude Medical prosthetic valve. The removed valve showed minimal cloth wear except for a small part of the strut The postoperative echocardiography demonstrated recovering of left ventricular function. To our best knowledge, this case presents the longest duration for a surgically explanted Starr-Edwards aortic prosthetic valve in Japan.

A 74 year old man with Canadian Cardiovascular Society class II stable angina was referred to São Lucas Hospital, Porto Alegre, Brazil, for cardiac catheterisation in May 2003. He had a past medical history significant for aortic valve stenosis, having previously been treated in August 1965 with a Starr-Edwards metallic valve prosthesis; the surgery was performed at the University of Indiana Hospital, Indianapolis, USA.
